FWIW:

1. The PUK and PUK2 codes are associated with your SIM so the make/model of your phone is irrelevant.

2. You should only need to use a PUK2 after typing in the wrong PUK several times (after typing in the wrong PIN several times.)

3. I had no problem getting the PUK code for my wife's SIM from SO CS after she accidentally locked herself out using the wrong PIN.

Got it to work. Just keep at it asking the rep for a PUK2 code not a PUK code. The manners of these reps are horrible.

The first rep gave me a lecture like I'm a horrible person for not using their shitty phone. The second rep was nice but didn't understand the problem. The third one insisted that its the PUK code until I kept saying its not at which point she put me on hold and came back with a PUK2 code like she really didn't want to give it to me (she had originally insisted that there is no PUK2 code and only a PUK code)

I tried entering the wrong SIM PIN too many times to my petro-canada sim when I was playing around with the "fixed dial" feature and now it says SIM blocked and asks for an unblock code. Anyone have a clue what to do? The phone still seems to receive and dial calls.

I just tried calling customer service and they said I have to call motorola (because I put the sim in a razr3 phone) wtf...
